Many people are confused by the idea that the amount of cortisol is different in healthy people and unhealthy people. The body produces an abundance of cortisol and stores the excess in the body as a fat-soluble substance called leptin. People with low levels of leptin can be hypothyroid (low body weight), and there are other causes, too. However, there are many people who are normal and healthy and whose bodies can't metabolize or absorb that leptin. This makes it possible for them to suffer from the effects of low leptin and other problems related to low cortisol. There is another type of low cortisol, known as hypothalamic low cortisol. This happens when the hypothalamus doesn't produce enough leptin or when the levels are too high. Low cortisol levels are found in people with metabolic syndrome. Low cortisol is often present at high levels in the brain and body and is associated with high levels of anxiety and depression.
